Telemedicine app development cost in 2026: build drivers and real run costs

What drives the cost of building a telemedicine app, and what it really costs to run: per-minute video prices from Twilio, Zoom and Amazon Chime SDK, BAA plan gates, a worked example, app store rules for consultations, and the Medicare, DEA and HIPAA dates that change what you build.

Key takeaways

  1. 01Build cost depends on scope (video, async messaging, e-prescribing, remote monitoring), EHR integration and the privacy and security work around them; run cost is mostly usage.
  2. 02Video APIs list at fractions of a cent per participant-minute: Twilio $0.004, Amazon Chime SDK $0.0017, Zoom Video SDK $0.0035 after 10,000 free minutes a month (read 23 Oct 2026).
  3. 03HIPAA BAAs are often gated: Twilio requires its Security or Enterprise Edition, AWS needs a signed BAA, and Zoom's Video SDK rate page doesn't mention one, so confirm with Zoom.
  4. 04Apple's guideline 3.1.3(d) lets real-time person-to-person services such as medical consultations take payment outside in-app purchase.
  5. 05Dates to plan around: DEA telemedicine flexibilities run to 31 Dec 2026, Medicare telehealth flexibilities to 31 Dec 2027, and the HIPAA Security Rule update is still a proposal (checked 23 Oct 2026).

Telemedicine app development cost depends on scope (video visits, async messaging, e-prescribing, remote monitoring), EHR integration and the privacy and security work around them. Running cost is mostly usage: video minutes, cloud, messaging and app store fees. Video APIs list at fractions of a cent per participant-minute, but the plan that includes a HIPAA business associate agreement can cost more.

Most cost guides that rank for this term give build ranges that contradict each other on the same page, treat video as a one-off feature rather than a monthly usage bill, and repeat a "Twilio Video is shutting down" claim that Twilio reversed in 2024. This guide doesn't publish build prices; Elgrith quotes each project from a written scope. Instead it explains what drives the build, then shows verified, dated run costs you can put into a spreadsheet today. Everything regulatory here is general guidance from public sources, not legal advice; your counsel confirms what applies to you.

What drives the cost of building a telemedicine app?

The same "telemedicine app" label covers very different products. These are the drivers we size in every scope:

DriverSimpler endComplex end
Visit typesScheduled one-to-one videoGroup visits, interpreters, on-demand queues, async (store-and-forward)
AppsOne patient app, web clinician consolePatient iOS and Android apps, clinician apps, admin portal
Clinical recordsVisit notes stored in your own systemTwo-way EHR integration over HL7 FHIR, SMART on FHIR launch
PrescribingNone, or referral lettersE-prescribing, including controlled substances where allowed
MonitoringNoneRemote patient monitoring with connected devices and alerts
PaymentsCard payment per visitInsurance eligibility checks, claims, subscriptions
Privacy and securityStandard encryption, access control, audit logsFormal risk analysis, BAAs with every vendor, penetration testing
MarketsOne countryUS plus UK (NHS DTAC) or other regions

The most expensive line is usually integration, not screens. A video visit that writes back into an EHR, checks eligibility and sends an e-prescription touches four outside systems, and each needs credentials, test environments and error handling. For the full scope of what we build, see telemedicine app development.

How much does video cost per minute in 2026?

Video is a usage bill. Here are list prices read on the vendors' pages on 23 October 2026:

VendorVideo priceRecordingBAA position
Twilio Video, Group Rooms$0.004 per participant per minuteParticipant recordings $0.004 per participant-minute; compositions $0.01 per composed minuteBAA requires Security Edition or Enterprise Edition
Zoom Video SDK, Pay As You Go10,000 minutes a month included, then $0.0035 per minute; minutes count per participantSeparateNot mentioned on the rate page: confirm with Zoom
Amazon Chime SDK$0.0017 per attendee-minute, billed in 6-second incrementsMedia capture priced separatelyListed as HIPAA eligible; AWS BAA required before PHI

Sources: Twilio Video pricing ("current as of August 2026"), Zoom Video SDK rates, Amazon Chime SDK pricing and the AWS HIPAA Eligible Services Reference (updated 15 Oct 2026).

Twilio Video is still a product. Twilio's changelog of 21 October 2024 says "we've reversed our earlier decision to retire Twilio Video in 2026." Guides that still tell you to migrate away are out of date.

We left Vonage out of the table because we couldn't confirm its current video price and HIPAA add-on on its own page. Ask Vonage directly if you're comparing it.

Which video APIs offer a BAA, and on which plans?

Under HIPAA, a vendor that handles protected health information for you generally needs a business associate agreement. The catch is that the BAA often isn't on the cheapest plan:

  • Twilio: its HIPAA page says "Customers wishing to sign a BAA with Twilio must have our Security Edition or Enterprise Edition." Price that edition in, not just the per-minute rate.
  • AWS: the Amazon Chime SDK is on AWS's HIPAA eligible services list, and you need a BAA in place with AWS before PHI touches it.
  • Zoom: the Video SDK rate page doesn't mention a BAA. Ask Zoom which plan includes one, in writing, before you build on it.

The same applies to every other vendor in the chain: cloud hosting, messaging, email, analytics, error logging and support tools. An analytics script that captures a patient's name in a page title can turn a free tool into a compliance problem. We design for that from day one, but whether a given setup meets HIPAA is a question for your compliance lead and counsel.

Worked example: what do 2,000 video visits a month cost?

Illustrative example, list prices read 23 October 2026. Assume 2,000 visits a month × 15 minutes × 2 participants = 60,000 participant-minutes.

VendorCalculationVideo per month
Twilio Group Rooms60,000 × $0.004$240
Zoom Video SDK (Pay As You Go)(60,000 − 10,000 included) × $0.0035$175
Amazon Chime SDK60,000 × $0.0017$102

If you record every visit on Twilio, participant recordings add another 60,000 × $0.004 = $240, and composing each visit into a single file adds 30,000 composed minutes × $0.01 = $300. That makes recording, not the call itself, the larger line. Many services don't record by default, and that decision has clinical, consent and storage implications as well as cost.

Two points from the example. First, at this volume video is a small part of the run bill; plan editions, cloud hosting, support staff and integrations usually cost more. Second, the BAA gate can outweigh the per-minute difference, so compare the full plan price, not just the rate.

Do Apple and Google take a commission on consultation fees?

This is a common budget surprise, and the stores treat it differently.

  • Apple. App Review Guideline 3.1.3(d) covers real-time person-to-person services "(for example tutoring students, medical consultations…)" and lets them use payment methods other than in-app purchase. See Apple's App Review Guidelines. Digital content, such as a paid wellness course inside the app, is a different matter.
  • Google. Google Play's Payments policy says Play Billing isn't required for "the purchase of physical services". It doesn't name healthcare, so don't assume telemedicine is exempt; check your exact model with the policy. Where Play Billing does apply, Google's service fee schedule changed for users in the US, UK and EEA on 30 June 2026; auto-renewing subscriptions, for example, are listed at 10% plus a 5% billing fee.

A subscription for unlimited messaging with a care team, a one-off video consult and an in-app course may be treated differently, so map each revenue line against both stores' rules during scoping. For store-specific work, see our Android app development and iOS app development services.

What drives the cost of EHR, e-prescribing and RPM integration?

  • EHR integration. HL7 FHIR APIs and SMART on FHIR launch are now the expected route in the US. The ASTP/ONC HTI rules (HTI-1 to HTI-4 final, updated 29 Apr 2026) push certified health IT towards standard APIs, and HTI-4 covers e-prescribing, real-time prescription benefit and electronic prior authorisation. Cost depends on which EHRs, whether you read or write, and each vendor's onboarding process.
  • E-prescribing. Usually through a certified e-prescribing network partner, with identity proofing for prescribers. Controlled substances add more requirements.
  • Remote patient monitoring. Device selection, pairing, data ingestion, thresholds and alert routing, and the clinical workflow for who responds.
  • Insurance. Eligibility checks and claims add clearinghouse integration and billing rules.

Each integration also needs a test environment, monitoring and someone to handle failures after launch, which is why ongoing costs matter as much as the build. Our healthcare app development team handles the wider patient and clinician side, and mobile app maintenance covers the care after release.

Which rules change what you build in 2026–27?

RuleWhat it saysDate (checked 23 Oct 2026)
DEA telemedicine flexibilitiesFourth temporary extension for prescribing controlled medications via telemedicine is "effective January 1, 2026 through December 31, 2026"Ends 31 Dec 2026; no fifth extension or final special-registration rule found as of 23 Oct 2026
Medicare telehealth flexibilitiesExtended by the Consolidated Appropriations Act, 2026 (signed 3 Feb 2026), per KFFThrough 31 Dec 2027
HIPAA Security Rule updateStill a proposed rule; final action listed for July 2027Proposed
ONC HTI-4E-prescribing, real-time prescription benefit, electronic prior authorisationFinal

Sources: Federal Register, DEA fourth temporary extension, KFF on Medicare telehealth and the regulatory agenda entry for the HIPAA Security Rule NPRM.

What this means for the build: if your service prescribes controlled substances, design the prescribing flow so an in-person requirement can be switched on if the DEA flexibilities lapse after 31 December 2026. Build strong security controls (encryption, multi-factor authentication, audit logs) as good practice now rather than waiting for a final HIPAA Security Rule update. We'll recheck the DEA date on 1 January 2027 and update this post.

Demand is real but not growing without limit: the CDC's Health E-Stat 120 (June 2026) found telemedicine use among office-based physicians "was lower in 2024 (80.0%) than in 2021 (86.5%)". Build for the visits that work well remotely, not for every visit.

Claims we checked

Claim you'll see elsewhereWhat we found (23 Oct 2026)
"Twilio Video is being shut down"Reversed: Twilio said on 21 Oct 2024 it will remain a standalone product
"Consultations must use Apple in-app purchase"Guideline 3.1.3(d) allows non-IAP payment for real-time person-to-person services such as medical consultations
"Telehealth is exempt from Google Play Billing"Not stated: the policy exempts "physical services" and doesn't name healthcare
"DEA flexibilities are permanent"No: the current extension runs to 31 Dec 2026
"HIPAA adds a fixed percentage to cost"We found no source for any such figure; it depends on scope and vendors

What ongoing costs should you budget after launch?

  • Video and recording minutes (usage, from the table above)
  • Vendor plans that include BAAs
  • Cloud hosting, storage, backups and logging
  • SMS and email reminders
  • EHR, e-prescribing and clearinghouse fees, which vary by partner
  • App store developer accounts and any applicable store fees
  • Security testing, monitoring and incident response
  • OS updates, store policy changes and bug fixes
  • Clinical and support staff time

How can you keep run costs down?

  • Price the BAA-eligible plan first, then compare per-minute rates within those plans.
  • Record only when there's a clinical or legal reason, and set retention periods; recording and composition can cost more than the call.
  • Use async visits where clinically appropriate: a photo or questionnaire reviewed later uses no video minutes.
  • Keep sessions short and well prepared with pre-visit forms, so the video time is spent on care.
  • Monitor usage per visit type so a pricing change or an unusual pattern shows up in days, not at the end of the quarter.
